Metabolic Surgery: New ray of hope for patients suffering from Type 2 Diabetes-Dr Amit Garg | Fortis Hospital (Mohali)

Dr Amit Garg, Chief Consultant, Bariatric & Metabolic Surgery, Fortis Hospital, Mohali addressing a press conference in Chandigarh shared regarding the new line of treatment for patients suffering from Type 2 Diabetes. He shared, “Till date, diabetes is poorly understood and medical treatment cannot claim a “Cure”. Within the last 20–30 years, Metabolic Surgery has become a common treatment for obesity in individuals both with and without Type 2 diabetes. According to the recent guidelines given by world Diabetes Associations (American Diabetic Association, International Diabetes Federation, Diabetes U.K.) Metabolic Surgery is recommended as the first line of treatment for Type 2 Diabetes in patients with BMI of > 37.5Kg/m2.The aim is to put it in remission, defined as normal blood sugar levels and no need for diabetes Surgery

He shared his concern regarding rising number in patients suffering from Type 2 Diabetes.“According to World Health Organization (WHO), India had 69.2 million people living with diabetes in 2015 and nearly 98 million people in India may have type 2 diabetes by 2030”, Dr Amit Garg further shared that Type 2 diabetes was called adult-onset diabetes since it was almost unheard of in children. But with the rising rates of childhood obesity, it has become more common in youth. Diabetes and obesity is catching the people young in Punjab. A national level nutrition survey has revealed that almost one-eighth of Punjab’s youngsters are suffering from the two ailments.”

Traditional 1st line treatment for type 2 diabetes include losing excess weight, eating a healthful diet, engaging in regular exercise; medications which help the pancreas produce more insulin or help the body to use the insulin that was produced efficiently. If these drugs don’t work, a person needs to take insulin. Often these traditional treatments for diabetes do not work and patients run the risks of long term complications. Even with good medical control of diabetes, some patients still develop cardiovascular complications, diabetic foot, neuropathy and nephropathy, retinopathy and other complications.

Answering the query regarding How metabolic surgery causes diabetes remission, Dr Amit Garg shared, “After Metabolic surgery, remission is observed in the days to weeks before any substantial weight loss has occurred. Metabolic surgery cures Type 2 diabetes by causing calorie restriction, hormonal changes such as reduced Ghrelin, increased GLP1 and peptide YY. These hormones stimulate beta cells in pancreas to stimulate increased secretion of insulin, provided enough beta cell mass is still present in pancreas. Persistent increased GLP-1 secretion increases beta cell mass in the long term. These gastro intestinal hormonal changes cause control of diabetes within a few days of the surgery.”

“Increasing evidence shows that metabolic surgery can help reduce a person’s risk for a number of serious diseases like diabetes, hypertension, heart disease, arthritis and cancer,” notes Dr Amt Garg.

“Metabolic surgery” is an ideal approach aimed at treating diabetes. Diabetes remission is observed in few days after surgery before any substantial weight loss has occurred. Therefore, metabolic surgery is also useful in non-obese Type 2 diabetes patients. Loop Duodenojejunal bypass with Sleeve Gastrectomy and proximal jejunal bypass with Sleeve Gastrectomy are novel metabolic surgical procedure to treat diabetes. Dr. Amit is the first one in North India to perform Loop duodenojejunal bypass with sleeve gastrectomy for Type 2 diabetes. He has more than 1000 surgeries and has many international publications to his credit.

Dr Garg also added, “All of the surgeries are performed laparoscopically, which means they involve making several small incisions that are closed with only a few stitches, resulting in fewer complications and a much quicker recovery time for patients.” “The health and quality of life benefits these patients receive from Metabolic surgery and other weight loss methods are tremendous,” says Dr. Garg. “They are more mobile, feel better about themselves, have less depression and have better family, social and work lives.”

Mrs Poonam Singh, 40 yr old Choreographer and video director from Punjabi Film Industry was suffering from diabetes though she was not morbidly obese. Post Metabolic Surgery, she is off all diabetes medicines and free from diabetes.

Manoj Grover from Delhi, 170 Kgs had a heart bypass a few years back with controlled diabetes, hypertension and sleep apnea. First day post metabolic surgery, he was off insulin and now his other medical problems are also resolved.

Dr Amit Garg added in his concluding message that although the genes you inherit may influence the development of type 2 diabetes, they take a back seat to behavioral and lifestyle factors.

  1. Control your weight: Losing seven to 10 percent of your current weight can cut your chances of developing type 2 diabetes in half.
  2. Get moving and turn off the television: Walking briskly for a half hour every day reduces the risk of developing type 2 diabetes by 30 per cent, according to a study.
  3. Tune up your diet: Choose whole grains and whole grain products over highly processed carbohydrates.
  4. Skip the sugary drinks, and choose water, coffee, or tea instead. Limit red meat and avoid processed meat; choose nuts, whole grains, poultry, or fish instead.
  5.  If you smoke, try to quit: Smokers are roughly 50 percent more likely to develop diabetes than nonsmokers, and heavy smokers have an even higher risk.
